If you need to order a Texas death certificate replacement (we CANNOT help you if you need to make corrections or changes to a death certificate) you will need to download, complete, sign, and scan/email back to us our rush Texas death certificate order form.

The three other required documents that you must email us to order are a signed and notarized state authorization form, a valid (can’t be expired) copy of your government ID, and a prepaid FedEx or UPS return shipping label so we can mail your newly issued certified death certificate copy back to you.
